Call for Letters of Interest - Continuing Care Demonstration Projects - EXTENDED


HSIF CONTINUING CARE DEMONSTRATION PROJECT

Request for Participation in First Nations Demonstration Projects in Continuing Care on Reserve in Alberta Region

1st Call Issued: February 20, 2103
Extension Issued: March 15, 2013
Letters of Interest Due: April 30, 2013
Demonstration Projects Commence: May 10, 2013
Projects End and are Evaluated Fall 2014 or Winter 2015

Call For First Nations, Tribal Councils, and other Treaty Entities to Participate in Demonstration Projects

The Management Team and Treaty 7 Management Corporation, who hosts the HSIF Continuing Care Project, are eager to join forces with First Nations across Alberta with the ultimate goal of creating a First Nations’ model(s) of continuing care for Elders, persons with disabilities, and persons with chronic illnesses.

If you have ideas on how to improve First Nations’ access to continuing care services and/or make the services people receive more culturally appropriate, then we would like to partner with you to make your ideas come to life! We know that many of you are already working with partners in order to maximize the financial and staffing resources you have associated with continuing care, and are still having difficulties meeting community demands with current resource allocations. Let us lend a helping hand!
